As you have mentioned that you have got 38k rank in viteee 2021 and you want to take biomedical engineering in category 1 . It is a bit difficult to predict the dynamics of vit because these things depends on certain factors like-
1- Number of candidates appearing in the entrance examination
2-Number of seats in college
3-Difficulty level of the paper
4-Previous years cut off
As per my knowledge ,you have good chances to get in category and at a worst case scenario in category 2.

But Cutoffs differs from year to year. The cut-off vary from year to year and depends on many factors like number of students appeared, difficulty level of the exam, number of seats available, category, etc.
If you can get Biomedical Engineering in Category other than Category 1 then you have to pay additional fees along with regular Tution Fees as per the fees criteria given by VIT.
