Can I take a print out of the Sample OBC-NCL certificate format provided in Information Brochure of JEE Advanced 2021 and make my certificate on the printed page itself? Or the authority in the Block or SDO office will provide me one?

No, you won't be eligible for jee advanced 2023 irrespective of whether you apply for/ attempt  JEE advanced 2021 or not.

This is because,

One has a total of two attempts for JEE advanced over two consecutive years starting from the year of appearing for 12th board examination i.

Since you have got 82.98 percentile your category rank will be somewhere above 1.5 lakhs and since you have ews category , your category rank will be somewhere between 27,000-28000. From the previous year cut offs of NIT silchar chances of your admission there are a bit low
