There are three channels of admission. Candidates will be admitted to IISER only through these channels.
Kishore Vaigyanik Protsahan Yojana (KVPY) channel
Joint Entrance Examination of Indian Institutes of Technology (JEE) channel
State and Central Boards Channel (SCB) channel
These channels will be referred to as KVPY, JEE and SCB respectively
Indian nationals, and students belonging to PIO or OCI category are eligible to apply provided they satisfy the eligibility criteria described below.
KVPY Channel
Candidates having a valid KVPY fellowship commencing from 2018-19 academic session are eligible to apply for admissions. Additional cut-off criteria may be applicable.
JEE Channel
Candidates in General category securing a rank within 10000 in the Common Rank List (CRL) of JEE Advanced 2018 are eligible to apply. For candidates belonging to reserved category (OBCNC, SC, ST, PD), their category rank should be within 10000.
SCB Channel
Students who have passed (10+2) level with science stream in 2017 or 2018 with marks equal or above the cut off percentage in their respective boards as given in the table below are eligible to apply. For the candidates belonging to the Non-Creamy Layer of the Other Backward Classes (OBCNC) and Persons with Disability (PD) categories, there is a 5% relaxation. The cut-off percentage for the Scheduled Castes (SC), the Scheduled Tribes (ST) and Kashmiri Migrants (KM) candidates is 55% in (10+2) level for all boards.
Students applying through SCB channel will have to appear in the IISER Aptitude Test which will be conducted at various centres across the country. To calculate your percentage of marks obtained, use the percentage calculation method adopted by your board.

IISER Aptitude Test is basically a screening test. The eligibility criteria for attending the test itself is kept above a certain standard that only the top students from each school board are eligible to apply. What the admission committee needs to do then is to conduct a test which will filter the best among the best. The test doesn’t have to be super hard as IIT JEE, because the eligibility criteria only makes the top fraction of students in the country eligible.
I have seen the question papers of 2010 and 2011 and they were definitely not as hard as the IIT JEE papers of the respective years. I don’t know how much the difficulty level of the test has increased over the years, but I believe it wouldn’t be as hard as the JEEAdvanced.

At undergraduate level there are three poasible ways of getting admission in iiser.
Give the kvpy exam(sx,sa,sb stream) and qualify it.
Qualify the jee advanced exam and be within top 10 000
If you are in top 1% of your respective central or state board (cut off percentage is mentioned on the iiser website) then you are eligible to give the iiser aptitude test. If you score well in the aptitude test and qualify the cutoff then you get the admission
Now to get in top iiser or iiser of your preference you need to have a good rank in kvpy jee advanced and top score in aptitude test.
P.s I have mentioned this for general category . There is some relaxation for obc sc and st provided
