hai saurabh, in general every doctor will know how to testify the student that he was medicaly fit so i think there will be no such formate the doctor will conducts the examinations and declare that you are perfect and gives you a certificate
The concerned authorities will be conducting BCECE 2018 counselling tentatively from the last week of July 2018. All of those candidates who qualify the entrance examination will have to register themselves for the counselling of BCECE 2018. After registration, the candidates will have to fill in their choices of colleges

As of now, the official counselling dates of BCECE have not been published by the authorities. The counselling is expected to start in July 2018, Registrations have to be open for the eligible candidates after which choice filling has to be done. Allotment of seats will be done according
