Since their is no official announcement of cutoffs by  NTA so you can't predict your eligibilty in appearing in JEE advance 2019,On my opinion you are not eligible for JEE advance ,you need to score above 95% percentile to confirms your  seat in IITs or NITs.

After considering the percentile mentioned by you, you may be eligible for sitting in JEE ADVANCED. But we cannot confirm

No definetly they will not take your physical test they only need your certificate of disability only as it is not any army or civil service exam so they will not at all check your disability they only need certificate , so that you will definetly get a seat in

Sakthi you need minimum of 75 % marks in aggregate subjects in both JEE Mains and JEE Advanced and 65% marks for SC/ST/PWD applicants .
