UPSEE 2013 Application Form Procedure:
1. UPSEE 2013 Offline Application Forms:
OMR application form with Information Brochure can be purchased on Cash payment of Rs 1000 (Rs 500 for SC/ST/Women Candidates) from Head Post Offices and Mukhya Dak Ghar (MDGs) of Uttar Pradesh, Delhi and Dehradun.
Affix a recent 3.5 cm x 4.5 cm Black & White photograph .
Send the duly filled OMR Application forms by Speed Post / Registered Post only to

Do not cut, scribe, tear, fold, staple, pin or tag the OMR Application form.
Do not use photocopy / reproduction / duplicate copy of OMR Application form.
Do not affix colour photograph in the form.
DO not get the photograph attested
Write in rectangular boxes for all the items of OMR Application Form with Black Ball Pen only
Darken the corresponding circles (O); with HB Pencil only.
Erase completely if you wish to change the option.
2. UPSEE 2013 Online Application Forms : Apply online at any of the following
Important: ONLINE applications, payment and confirmation pages can be viewed only on Internet Explorer 7 and above.Submission of Online Application Form:
Before submitting the form please review the details entered by you using the “review button”.
A unique registration number will be generated on submission of the form.
Please note this registration number to be used in all future correspondence with the university.
Payment of Examination Fees (Online Application forms): Use any of the following to pay the fees:
1. Bank challan:
Click here to generate and print the challan in triplicate with your credentials.
Print it and use it to pay the examination fees to the designated bank after 24 hours.
After payment, make sure that the transaction number is filled up correctly at the space provided on the challan by the bank.
Log in to the website a day (24 hours) after the payment.
Fill up the registration number and the transaction number

2. Credit/ Debit card/ Internet banking:
Keep the credit, debit card / internet banking details ready.

Note: Only Visa/Master cards accepted
Click here to print the confirmation page after payment is over
Take two printouts of the confirmation page (one has to be kept as the candidate’s copy)
Affix a recent passport size photograph
Put your signature and the left thumb impression on the designated space in the confirmation page
The signature of the parent/ guardian signature must be at the space provided on the confirmation page.
Send the duly signed completed confirmation page along with copy of challan (in case of the payment through bank challan) to reach
